Birth Plate: enter: -first name, -date of birth, -time of birth, -birth weight (pounds and ounces only!), -city and -state of birth Please note, birth weight must be in pounds and ounces. Please do not include any fractions.

Church Plate: enter -city, -state, -type of event (christening, wedding), -date and -time , -name of couple or -child

Anniversary Plate:
couples -first and -last name, -children's names (up to 9) -date of marriage, -choose gold or silver rim and -year for anniversary
